Has a black belt martial artist ever lost to someone with a lower belt from the same style?

And if so, why did he/she lose and what style of martial arts was it? Also, how often does this occur? I’m interested to know if this has occured in Brazilian Jiu-Jitsu.
Remember, both fighters are using the same martial arts style in competition or sparring.
